7:00-8:15 a.m. Shotgun start at 8:30 a.m.

| Format: |
Scramble - Four golfers per team, up to three flights. The
flights are determined by taking the total number of golfers
and dividing by three. These three groups will represent the
first flight, second flight, and third flight. The Great Bear Golf
& Country Club golf pros will be responsible for ranking the
flights and teams.

| Entry Fee: |
Includes green fees, carts, group photos, continental breakfast
and luncheon.
Option 1: $120 per person
Option 2: $145 per person with raffle ticket

| Raffle: |
$25 per ticket (sales limited to 300 tickets, top prize $1,000)
*Raffle participants need not be present to win.* |
Luncheon and Recognition Ceremony immediately following the
golf tournament at approximately 1:30 p.m. at
Luncheon Only: $40 per person

Chicken Piccatta and Baked Lasagna
Tossed Salad, Side Dish, Vegetables, Dessert
Drinks (fountain soft drinks, coffee, hot tea and decaf) Included
